If you or a family member received a cancer diagnosis while living in Ladera Ranch, your report helps build an accurate community registry. Takes 5 to 10 minutes; any question can be skipped.
How your information is used
- Purpose: reports help us build an accurate community cancer registry and support our advocacy, including our request for a California Cancer Registry analysis.
- Aggregation: reports may be aggregated and shared with public health experts or agencies. We do not publish individual reports.
- Voluntary: submitting a report is entirely voluntary, and you may skip any question you don't want to answer.
- Registry coordinator: reports go to the registry coordinator, not a public database.
- Confidentiality: personally identifying details are never shared publicly.
- Removal: you can request that your report be updated or removed at any time by contacting info@ochealthwatch.org.
